Not a problem. The next time you’re at an OSU facility for an appointment, let the staff know you’d like to enroll in MyChart. They can get you signed up instantly.

Forgot your MyChart password? Reset it in seconds.
- Go to the MyChart login page
- Click “Forgot Password?”
- Enter your MyChart username
- Enter the email on file and your date of birth
- Click Next and follow the reset instructions emailed to you
